Homologation - Required documents

Certain documents are required in order to open a Homologation Assistance Service (HAS) file.

• The Homologation form. (1)
  
  
• A copy of the judgment to be modified.
  
  
• A copy of the support determination form used to calculate the support payments.
  
  
• Individuals who may be financially eligible for gratuitous or contributory legal aid must provide poof of their employment and bussiness income as well as proof of their income from property and details about the assets they own. You can consult the legal aid eligibility thresholds under the « Legal Aid » tab in the section entitled « Am I financially eligible? ».

Solution 1 - Save the link as

1- Position the cursor over the above link, right-click the mouse and select «Save Link As ...» to save the PDF file on your desktop.
2- Open the PDF file from the location saved.

Homologation form

The browser will display the file in the bottom of the screen when it will be downloaded.


To open it, go to the Download directory. Right-click and select «Open with » and « Adobe Acrobat Reader » .
